You may have heard the news in the last two days about the Deepwater Horizon drilling rig which caught fire, burned for two days, then sank in 5,000 ft of water in the Gulf of Mexico.  There are still 11 men missing, and they are not expected to be found.

 INCIDENT

While operating a RAM car (rubber-tyred mobile plant) in an underground coal mine, the driver heard a bang and  noticed a distinctive smell and white smoke-like substance coming from the rear of the machine. A brown smoke-like substance followed.

INCIDENT

During a practical demonstration of self-rescue units (oxygen-generating breathing apparatus), a member of a mine rescue team attempted to activate and wear a unit. The team member found that the mouthpiece and breathing circuit had been contaminated with a yellow powder and was unusable.
